Do I need an appointment? expand_more

No. Both Three Peaks locations are true walk-in clinics, so you can be seen without an appointment during our daily 8 AM to 8 PM hours. You can also book online to hold your spot and shorten your wait.

What does a visit cost if I don’t have insurance? expand_more

We offer clear, affordable self-pay pricing, and you’ll know the cost before you’re seen. Common upfront prices include DOT physicals at $40 and sports physicals at $110. Ask the front desk about self-pay rates for any service.

Which insurance plans do you accept? expand_more

We accept most major plans, including Aetna, Anthem, Cigna, United Healthcare, Humana, Medicare, Medicaid, HealthChoice, and Rocky Mountain Health Plans. See our Insurance page or call to verify your coverage.

When should I choose urgent care instead of the ER? expand_more

Urgent care is ideal for non-life-threatening problems: flu and colds, sprains, minor fractures, cuts needing stitches, infections, and rashes. For chest pain, severe bleeding, difficulty breathing, or major trauma, call 911 or go to the nearest ER.

Do you offer immigration (USCIS) physicals? expand_more

Yes. A USCIS-designated civil surgeon completes and signs your Form I-693 on site. Immigration physicals are by appointment so we can allow time for testing and documentation. Hablamos español.

Are you open on weekends and holidays? expand_more

Yes. Both locations are open every day from 8 AM to 8 PM, including weekends and holidays.

How long does the process take? expand_more

Most patients finish in two visits. Lab results take a few days, so the full process is usually complete within a couple of weeks depending on testing.

Can I walk in for an immigration physical? expand_more

No. Immigration physicals are scheduled in advance so we can allow time for testing and documentation. Please call to book.

What if I’m missing vaccination records? expand_more

If records are incomplete, additional testing or vaccinations may be needed. We’ll guide you through exactly what USCIS requires.

Who signs my USCIS paperwork? expand_more

Only a USCIS-designated civil surgeon can complete and sign Form I-693. We provide that service on site to ensure compliance and accuracy.
